How do I register with a GP?

• Find a GP and more information at www.nhs.uk/register
• Telephone your local GP Surgery and ask to be registered as a patient
• Complete the New Patient Registration Form online
• Or download the form and send it back to us. Please follow steps below:

Download the form (word file) below and email us the following croft.prescription@nhs.net

• Duly filled registration form
• One photo ID (Driving license or Passport)
• One proof of Address (Driving license or Passport)
